Before You Begin

It is important that you read all of the documentation and instructions in this application.

Please ensure that you are using a computer with printer capabilities as you will be asked to print out your applicant checklist at the end of the application.

This application to the Master of Physical Therapy program may take a typical applicant 15-20 minutes to complete.

You will be asked to provide your student number (U of S) and your prerequisite grades so you may want to have your academic record handy. You will also be asked for the mailing address for your next of kin.

Any personal information you provide will be used for the purpose of this application only and will be held in confidence. Please make sure that you have reviewed the current requirements and prerequisites before starting this application.

Students entering the School of Physical Therapy are required to complete a Criminal Record Check (CRC), including a Vulnerable Sector Check (VSC) as a pre-requisite to admission to the program. The CRC/VSC should NOT be done until admission to the program is confirmed. Documents that are required to be sent in to support your application (as identified in your applicant checklist) can be sent to:

Please note that students who have completed all of their studies at the University of Saskatchewan do not have to request academic transcripts be sent to the School. If you attended any university other than the University of Saskatchewan, transcripts will need to be uploaded by the applicant to the Graduate Admissions Document Upload page.
